HOW TO ENROLL

Step 1:

Contact Maryland Access Point (MAP)

Call the Baltimore City Maryland Access Point at 410-396-2273 to speak with an aging services counselor.

Step 2:

Ask for a LEVEL ONE SCREENING for Adult Medical Daycare

Step 3:

Ask for ADULT DAYCARE ASSESSMENT

The call time for phone calls may be up to or more than 45 minutes. Be patient and get your time on the phone with the processing agent. do not hang up.

*What is your assessment date?

When is your nurse coming to do the assessment of your home?

IMPORTANT NOTE: Your screening and assessment is not complete until you receive an in-home assessment from the Baltimore City Department Of Aging Assessors. It is very important to receive the date as well as the assessment to keep your phone number current. Any changes in housing have to be updated so that the Baltimore City Department of Aging can contact you on the date of your assessment and any follow-ups. Please also give them an emergency contact number.

Step 4:

Call for a Case Manager.

Contact a CMA of your choice. There are five Case Management Agencies that are authorized to provide case case management services for the MDSCW:

Contact Kingdom Elements to schedule a start date and transportation.

We look forward to a personalized plan of care. Coordinate with the center's director of nursing to review the personalized care plan (covering medication management, dietary needs, and therapy schedules).

Follow up within 7 days this process should take no longer than two weeks. You have the right to be accessed within 14 days. Keep calling 410-396-2273 to ensure you are accessed.
