Required Documents to Enroll in Kingdom Elements Adult Medical Day Care
A Kingdom Elements AMDC staff member will assist you and your family through the admission process, but it is helpful to be prepared. If your loved one is currently in the hospital, some of the required items may be coordinated by hospital staff. Otherwise, please make an appointment with your primary care physician to discuss the following items and help prepare for admission.
1. Signed Physician’s Order for Admission to Kingdom Elements AMDC
A physician must confirm that the participant requires Kingdom Elements AMDC. This order authorizes the participant’s admission and is similar to a prescription for services.
2. Signed Physician’s Orders for Medications and Treatment
To provide appropriate care, the medical team at Kingdom Elements AMDC must receive current physician orders for all medications, treatments, and other necessary medical services.
3. Medical History and Physical (H&P)
Kingdom Elements AMDC requires current medical information about each participant. The participant's primary care physician should complete a current physical examination and medical history.
4. State-Required Assessment (Level of Care)
Maryland requires participants to have a Level of Care assessment completed before receiving Kingdom Elements AMDC services. This assessment may be completed through the appropriate hospital, local health department, or other authorized agency.
5. Transferring from Another Medical Adult Day Care Program
Participants transferring from another Kingdom Elements AMDC program may not be required to complete a new Level of Care assessment. A Voluntary Consent to Transfer (VCT) form or other required transfer documentation may be completed and submitted to the admissions staff.
6. Negative Tuberculosis (TB) Test or Chest X-Ray
Participants must provide documentation of a negative Tuberculosis (TB) test or other acceptable medical clearance, such as a chest X-ray when appropriate. This requirement helps protect the health and safety of all participants and staff.
7. Properly Labeled Medications
Kingdom Elements AMDC can only administer medications that are provided in properly labeled containers from the pharmacy. Medications will not be accepted if they are stored in plastic bags, have handwritten labels, are unlabeled, or do not match the current physician's medication orders.
8. Hospital Discharge Paperwork or Recent Medical Records
To provide comprehensive and coordinated care, participants should provide recent hospital discharge paperwork, medical records, or other relevant health information. If these documents are not available, please inform the admissions staff, who may be able to assist with obtaining the necessary information.
9. Completed Admissions Paperwork
Admissions paperwork may be completed before or on the day of admission. In many cases, the required forms are completed at the facility with the assistance of a Kingdom Elements AMDC staff member.
